I confirm that I understand the Cotswold retreat includes ten nights at Notgrove Holiday Estate, regional excursions, entry fees, all meals except three lunches, creative workshops, and group transportation to and from designated location near Heathrow Airport. I acknowledge that airfare, three lunches (plus the arrival day lunch), tipping, art supplies for personal projects, and personal items are not included.

I confirm that I understand I should not book airfare until I receive confirmation from the host. I acknowledge that a valid passport (six months beyond the reentry date to the U.S.) is required. I am also aware that travel insurance is highly recommended, and pre-existing condition coverage may require purchase within 14 days of group confirmation.

I confirm that I understand the Cotswold retreat pricing: Double Occupancy is $3,750, Single Occupancy is $3,950, and a non-participating guest is welcome with a full participant at a $200 discount. I acknowledge that a $600 non-refundable deposit is due at registration to hold my space, and that all payments are non-refundable once the trip is confirmed. If the retreat sells out, a waitlist will be created, and if I cancel after the retreat is sold out and my spot is filled, I will receive a refund minus a $200 processing fee.

By submitting this registration form, I agree to release and hold harmless Unity Arts Ministry, Jen Hutchins, Melissa Goslin, and their individual and collective heirs from any losses, delays, damage, or injury I may incur due to accident or negligence of any cause. I accept full responsibility for all risks inherent in international travel. I also understand that if I cancel for any reason, I will forfeit all payments made.
